Mayo Clinic, Rochester, Rochester Methodist & St. Mary's - You must have prior authorization from Sanford Health Plan to make an appointment at Mayo Clinic, Rochester or St. Mary's. Please contact the Health Services Department at 605-328-6807 or 800-805-7938 for authorization should your physician refer you to the Mayo Clinic for services.

If you choose to go to a non-participating provider when access is available, your claims will be paid according to your out-of-network benefits.
If you are a Minnesota resident and have the Preferred One logo on your ID Card and do not have access to a Sanford Health Plan participating provider, you may access the Preferred One* Network. Please log on to www.preferredone.com to locate a participating provider outside of the Sanford Health Plan service area. If you choose to go to a non-participating provider when access is available, your claims will be paid according to your out-of-network benefits.
If you are an Iowa or Nebraska resident and have the Midland’s Choice logo on your ID card and do not have access to a Sanford Health Plan participating provider, you may access the Midlands Choice* Network. Mental Health - If you are having difficulty obtaining an appointment with a mental health provider, Sanford Health Plan members may contact the Behavioral Health Triage by calling 605-328-4777 or toll-free at 888-996-4673.
Foreign Language - If you would like a listing of providers who speak languages other than English, please contact Provider Relation's toll-free at 800-601-5086. A listing will be sent directly to your home upon your request.
Emergency - Members are responsible for seeking emergency care at the nearest Plan participating emergency facility whenever possible or call 911 for assistance. In the event an ambulance is used, direct the ambulance to the nearest participating emergency facility unless the condition is so severe that you must use the nearest emergency facility. State law requires that the ambulance transport you to the hospital of your choice unless that transport puts you at serious risk.
