Diabetes Disease Management Program

What is it?
The Diabetes Disease Management Program is a program designed for diabetic members to provide them with information about diabetes and lifestyle. The information includes topics such as blood sugars, eating right, medications, exercise, recommended tests and more. This program does not replace the direction and guidance of your healthcare practitioner; however, it compliments and reinforces the information received in the office. The goal of the program is to provide tools to educate our members on promoting improved health through better prevention, detection, treatment and education.

Who is eligible?
All Sanford Health Plan and Sanford Health Plan of Minnesota members who have a diagnosis of diabetes.
