| What is it? The Hypertension Quality Improvement Activity provides information to our hypertensive members regarding hypertension and their lifestyle. These mailings are sent throughout the year and the goal of the activity is to increase the number of members with controlled high blood pressure. Who is eligible? All Sanford Health Plan, Sanford Health Plan of Minnesota members who have a diagnosis of hypertension.
