Health Screens
Goal
Provide screens to identify individuals at risk for disease.

Health Risk Assessment
Goal
Identify modifiable health risk behaviors, promote healthy lifestyles and offer recommendations to deliver proactive wellness and healthcare services.

  • Personal Wellness Profile Questionnaire - A self-directed questionnaire requesting information regarding personal behaviors that influence health to be complted prior to the screening. Health behavior categories include health information, physicial activity, eating practices, alcohol, drugs and smoking, stress and coping, social health, safety, medical care and health view.
  • Personal Wellness Profile - A confidential report evaluating the employee's present health status and offering recommendations for the development of a personal wellness program.
  • Executive Summary - A summary offering recommendations the employer can utilize to address organizational health care concerns and the cost-savings of implementing wellness and prevention programs.
  • Recommended Action Plan - A report offering recommendations for implementing programs to address identified health care concerns.
  • Annual Review - A report reviewing implemented actions at the conclusion of a twelve-month period leading to the preparation of a revised Recommended Action Plan for the upcoming year. Progress reports evaluating current Personal Wellness Profile and Executive Summary are included.

Goal
Educate individuals on the essential components of a healthy lifestyle.

Incentive Programs
Goal
Encourage individuals to start and/or acquire new healthy lifestyle skills to ensure continued success in wellness efforts.

Health Education Program 
Goal
Create awareness of risk factors, lifestyle behaviors and disease. Educational materials will be distributed to those at risk for disease development. Education will be specific to the risk factor or lifestyle behavior and focus on:

  • General Information
  • Nutrition
  • Prescription Medications / Dietary Supplements
  • Exercise / Stress Management 

Lifestyle Modification Program
Goal
Develop, monitor and evaluate lifestyle changes focused on decreasing risk factors for disease. The program includes:

  • Individualized action plans
  • Action plan review and modification
  • Follow up screens
  • Screen result tracking
